Overview

The Department of Health (DoH) is undertaking an Outpatient Reform Program as a system-wide initiative across DoH Divisions and Health Service Providers. The program aims to improve the outpatient journey for Western Australian health consumers and their carers, and support Western Australian clinicians.

Why your views matter

As a General Practitioner (metropolitan or country WA) you are the main external referral source to metropolitan based WA Public Hospital Specialist Outpatient Services i.e. medical and surgical services provided by doctors to non-admitted patients. Your feedback is vital to inform improvements in outpatient management processes. The following survey is a joint initiative between DoH and the WA Primary Health Alliance (WAPHA).

Survey Instructions

  • The survey should take approximately 15 minutes to complete.
  • There are 5 sections, each with an average of 6 questions.
  • The majority of questions require checkbox responses, with an opportunity to provide additional comments.
  • Your responses will remain confidential. Survey results will be shared with you - results will be aggregated and not identifiable by individual GP or practice.
