Consultation on draft advance care planning guides for consumers and health professionals

Overview

Advance care planning (ACP) is a voluntary process of planning for future health and personal care whereby the person’s values, beliefs and preferences are made known to guide decision-making at a future time when that person cannot make or communicate their decisions.

The End of Life Care (EOLC) Program within the WA Department of Health has developed 2 guides to advance care planning:

• Health professional guide to advance care planning
  This Guide provides an overview of the health professional role in advance care planning and links to existing resources for more details and guidance.
  It will replace the current documents A Guide for Health Professionals to Acts Amendment (Consent to Medical Treatment) Act 2008 and Advance Care Planning: A step-by-step guide for health care professionals assisting patients with chronic conditions to plan for care towards end of life.
• Your guide to advance care planning in Western Australia: A workbook to help you plan for your future care
  This Guide provides an introduction to advance care planning and includes activities to help individuals start the advance care planning process.
  It will replace the current Advance Care Planning: A Patient’s Guide.

The draft Guides have been developed by Subgroups of the Advance Care Planning Education Reference Group who are tasked with overseeing a review of all WA Health ACP resources and information. Their work is informed by the recommendations from the My Life My Choice Report and a subsequent Ministerial Expert Panel report on Advance Health Directives and aim to enable Western Australians to more effectively engage in advance care planning. 

Why your views matter

The EOLC Program is seeking feedback into the draft Guides via the completion of a survey.

We encourage all members of the WA community to have their say by completing this online survey. In particular, we are seeking feedback from individuals with an interest in ACP and health and care providers involved in ACP with patients/clients.

The information collected by this survey will inform the finalisation of the Guides and support their implementation across the State.

About the survey

The survey is expected to take about 20 minutes to complete. All responses to this survey will remain private and confidential.

Before commencing this survey, please ensure you have downloaded the draft Guides that you wish to comment on (see ‘Related Documents’ below survey link) and have them available to refer to. You will have the option of providing feedback on one or both of the Guides.
