Why your views matter
Anyone with an interest or involvement in advance care planning is invited to attend a workshop to have their say on the development of the revised WA Advance Health Directive (AHD) form.
The End of Life Care Program within the WA Department of Health is developing a revised AHD form following recommendations from the 2018 My Life My Choice Report and the subsequent Ministerial Expert Panel report on Advance Health Directives.
An Advance Health Directive is a legal document that enables you to make decisions now about the treatment you would want - or not want - to receive if you ever became sick or injured and were incapable of communicating your wishes. The revised AHD template is aimed at encouraging consumers to more effectively engage in advance care planning and put their health care preferences in writing.
