Closed 23 Jun 2023

Opened 12 Jun 2023

Before completing this survey, you will need to read the ‘Draft Chronic Conditions Outcomes Framework June 2023' (hyperlink), approximately 20-30 minutes.

Chronic conditions are the leading cause of illness, disability and death in Australia, and the prevalence is rising. People living with or at risk of chronic conditions can face significant physical, psychological and financial stress impacting their quality of life. Yet, in Western Australia, there is no current state-wide guiding document addressing chronic conditions management.

Commencing in mid-2022, the WA Department of Health has been developing a Chronic Conditions Outcomes Framework. The purpose of the framework is to guide localised and collective action to improve the prevention, early intervention and management of chronic conditions in Western Australia. This Framework aims to:

  • improve program and service planning and decision making, coordination and delivery
  • guide investment through commissioning and procurement
  • provide guidance for how community-level programs and services can align with state-level priorities and outcomes.

Integral to the project has been extensive consultation with a variety of stakeholders. To date this has included an online survey, a series of online workshops, targeted stakeholder interviews and 3-full day workshops with a 41-person deliberative stakeholder panel, helping to shape the direction and content of the Framework.

The final draft is now complete, and we are seeking feedback via survey. This survey will be the final consultation opportunity and aims to identify any key gaps before release of the final Framework.  Responses will contribute to the final version of the Framework.

You can choose to remain anonymous when responding to this survey. There will be an option to provide your details if you would like to be contactable to provide further detail regarding your responses or to be notified upon release of the Framework.

The survey will take approximately 10 minutes to complete. Reviewing the Framework will take approximately 20 minutes.

This survey and the draft Chronic Conditions Outcomes Framework can be made available in alternative formats on request to healthpolicy@health.wa.gov.au.

Why your views matter

Chronic conditions cover a diverse range of disease groups, people, communities and experiences. It is important that we obtain diverse perspectives including from, but not limited to, people with lived experience, carers, health professionals, social and community workers, and corporate staff (policy, program, data etc.).

Your perspectives will help us to refine the critical changes that need to occur to reduce the impact of chronic conditions on our communities.
