Kidney Supportive Care: Establishing a Model of Service Delivery in Western Australia (previously known as Renal Palliative Care Services).
The drafted Kidney Supportive Care: Establishing a Model of Service Delivery in Western Australia provides an updated framework to the 2012 Pathway for Renal Palliative Care Services in Western Australia, reflecting current evidence and needs of patients living with advanced chronic kidney disease (CKD).
This updated framework aims to:
- Propose a coordinated, state-wide delivery of Kidney Supported Care (KSC) services in Western Australia (WA).
- Serve as foundational guide for health policy development.
- Support strategic statewide service planning and coordination.
- Provide recommendations for phased implementation of KSC services across WA.
Scope:
The model applies to all adults with advanced CKD, including those on dialysis, individuals with a kidney transplant, as well as those who choose conservative kidney management (CKM).
Out of scope:
This model does not currently include:
- Children and young adults under 18 years of age.
- Transition planning between paediatric and adult kidney services.
- Detailed implementation guides or operational frameworks.
- Funding models.
Background:
The draft Kidney Supportive Care Model: Establishing a Model of Service Delivery in Western Australia has been developed by the Health Networks Directorate (HND) within the Department of Health WA, under the leadership of the HND Renal Clinical Lead. The development process has been informed by current best practice, stakeholder consultation, and lessons from current service delivery across WA.
KSC is an established part of routine kidney care (integrated comprehensive kidney care). This model promotes the integration of renal and palliative care specialties, with strong links to primary care and community-based services.

We welcome feedback from anyone with an interest in Kidney Care in WA. We are particularly keen to hear from:
- healthcare professionals (clinical, allied health or administrative)
- health service providers, planners and administrators
- people living with CKD
- family members, carers, or representatives of people with CKD.
