Mapping of Community Based Health Services for Respiratory Conditions in Western Australia

Overview

The Respiratory Health Network is undertaking a mapping exercise to identify all community based services currently available for patients who have a chronic respiratory condition or are at risk of developing a chronic respiratory condition.

For the purposes of this survey, a community based health service is defined as a service delivered in the community setting which focuses on the prevention, maintenance and improvement of the health status of its community members.

The term service is inclusive of:

  • Activities
  • Programs and/or
  • Initiatives

The survey will take you approximately 20 minutes to complete. You can remain anonymous if you choose, however we ask that you provide the name of the organisation and service in order for the mapping exercise to be comprehensive and useful, and in case we need to contact you to obtain more information about the service details that have been provided.
