Overview
Thank you for your interest in contributing to the development of the Western Australian (WA) State Dementia Action Plan (SDAP).

State Dementia Action Plan
In 2025, the WA Government committed $1 million for the development of a State Dementia Action Plan (SDAP) to address the priority actions of the National Dementia Action Plan 2024-2034 (NDAP) in the WA context.
Consumer and carer consultation survey
The Department of Health has created this survey to seek feedback from consumers and carers on the priority areas that WA should be focusing on in the SDAP.
The aim of this survey is to identify:
- improvements to access information and services for dementia
- key messages to support people to live with dementia at home.
This consultation survey is presented in 2 sections:
1. Dementia experience – we are asking you to identify the service gaps and opportunities for improvement for the patient pathway for dementia.
2. Public communications campaign – we are asking you what messages are central to support people to live with dementia at home.
Why your views matter
The results from this survey will be used to inform the development of the SDAP.
The survey will close on 16 January 2026
