Why your views matter
Consumer and carers with an interest or involvement in advance care planning are invited to attend a workshop to have their say in shaping the revised WA Advance Health Directive form (the revised AHD form).
The WA Health End of Life Care Program within the Department of Health WA is developing a revised AHD form following recommendations from the My Life My Choice Report and the subsequent Ministerial Expert Panel report on Advance Health Directives.
An Advance Health Directive is a legal written document, containing your decisions about your future medical and/or surgical treatment, including life sustaining measures and palliative care. The revised AHD template is aimed at encouraging consumers to more effectively engage in advance care planning and put their health care preferences in writing.
