Road Testing the Revised Advance Health Directive

Overview

An Advance Health Directive (AHD) is a legal document that enables an individual to make decisions now about the treatment they would want, or not want, to receive if they became sick or injured and were incapable of communicating their wishes. More information on the current Western Australian (WA) AHD template is available for background information.

The draft revised WA AHD template is being developed by the End of Life Care (EOLC) Program within the WA Department of Health. The project is informed by the recommendations from the Ministerial Expert Panel on Advance Health Directives: Final Report (August 2019) and aims to enable more Western Australians to effectively engage in advance care planning. 

During the broad consultation phase in August this year, the EOLC Program sought input into the content to be included in the revised AHD template from the WA community. The feedback has been analysed and used to shape a draft revised AHD which is now ready to be tested. 

Why your views matter

The EOLC Program is seeking your assistance with testing the draft revised AHD via the completion of a survey.

We are seeking input from people who have completed or are considering completing an AHD, and health professionals who assist their patients and clients in the completion of an AHD.

The survey will gather your feedback on the usability of the draft AHD. The information collected will inform further revisions before the AHD is finalised.

About the survey

The survey is expected to take about 15 minutes to complete. All responses to this survey will remain confidential.

Before commencing this survey, please download the draft AHD (see ‘Related Documents’ below survey link) and review the draft AHD by working through the form as you would if you were completing it for yourself or assisting someone to complete it. Please have the draft AHD available to refer to when you complete the survey questions.
